PacifiCare Behavioral Health and U.S. Behavioral Health Plan, California have announced a merger effective in 2010. After the merger of these two plans they will be recognized under the new name of OptumHealth, Behavioral Solutions of California. The merger and name change will have no impact on member benefits, treatment or interaction with their Customer Service department, and their website, www.liveandworkwell.com, will remain the same.

The Vacation Checks have been prepared and are queued to be mailed on Saturday, January 29, 2011 for hours worked between 8/1/2009-7/31/2010 and paid through 12/31/2010.
If you are expecting a vacation check and do not receive it by Wednesday, February 9th, you may email BenefitServices@carpenterfunds.com or call the Fund Office at (888) 547-2054.

Effective January 1, 2009, Indemnity Medical Claims will be processed at the Trust Fund Office. Claims should be mailed to Anthem Blue Cross, PO Box 60007, Los Angeles, CA 90060-0007. Indemnity Medical claims incurred prior to January 1st will still be processed by ATPA.

Effective January 1, 2007, your Percentage of Contribution Benefit Credit will be based on the Contributions required to be made to the Fund on your behalf for each calendar year multiplied by the applicable Percentage of Contribution Factor. More>>
